Casting on in the middle?

Hi all, I'm still hard at work, experimenting with various aspects of making doll clothes for my SIL's Madame Alexander dolls. I'm working on a cute little lavender sundress, sleeveless, with an off-the-shoulder ribbed neckline. The piece is knit from the top down.

I fudged my way through a directive but would like to know the correct way to do it for next time: I had to cast on some stitches in the middle of the piece for the bottom part of the armholes, so I was working sort of in thin air. What is the correct way of casting on to bridge a gap like this?

I'll be up north of here this weekend staying with friends while I go to the Wool Festival, and they don't have a working computer, so I'll have to try to be patient and read any kind answers upon my return home on Sunday.
