Semantics. I think. Any time I see "pick up and knit" I do this and so far I've produced reasonable finished goods:

Stick your right-hand needle through the first chain stitch on the gusset. Pull the yarn through and make a loop on the right-hand needle. Repeat. Repeat. Repeat...

One gusset picked up.

Knit across the instep.

Stick your needle through the first chain stitch up the side of the gusset. Pull the yarn through and make a loop on the right-hand needle. Repeat. Repeat. Repeat.

Second gusset picked up.

Knit half of the remaining heel stitches onto the second gusset needle. Transfer the other half of the remaining heel stitches onto the first gusset needle.

• Knit one round plain. Knit a decrease round: If the beginning of the round is at the back of the heel knit up the first gusset to 3 stitches of the end; k2tog, k1.

Knit across instep

Knit one on second gusset. SSK or sl1k1psso or whatever to make a left-leaning decrease.

Repeat from * until you're back to your original number of stitches cast on for the sock.

Make the foot. Make the toe. Done.
