issues with quilting density?

The quilt police will fall upon your house in one fell swoop and confiscate all your quilting materials. Or... Whats more likely to happen is that the batt will pull apart more during use and washing. Diana

So, I'm curious -- what really can happen if you don't quilt as "densely" as specified for the particular batting you're using? For example, if it says to quilt every 4" and I only have 6" spacing, what problems might arise? Is this mostly an issue when washing later?

Yes, everything Diana said. Also have a look at very old quilts...the ones that have held up best over the years tend to be heavily quilted.

As was told to me once..."Once you "understand" the rules...then you'll know which ones you can break"

In addition, if you are planning to hang the quilt, there may be issues with the surface of the quilt sagging or rippling in the open areas due to lack of support.
