Can anyone point me to a template to make a envelope or jacket for a CD/DVD? Not the label that goes inside a jewel case, but the "sleeve" that is used instead of the jewel case.

Thank you Luna. After looking at the templates M-C posted for the jewel case I'm wondering if I just need a 5.25"x10" strip of paper folded in half with the two sides glued close. I might have to give that a try. I don't know why I was thinking it was more complicated than that. Duh!
