But I wish there were "other" with a blank. Sometimes the answers did not apply to me in their why I do or don't do things.

Done! I agree with Melissa. There were some questions I answered that just didn't fit my thoughts. ;o) I understand the reason for limiting the questionare though.
Good luck to your friend with her report.

Done! I agree with Melissa and Tammy I wish there was a box marked "other" also on the various questions. But overall not a bad survey.
