I have just starated making some patches for a small business here . It's an easy process and I'll try to explain it:

You will need: a "template" the size and shape of the finished patch. water soluable stabilizer tear away stabilizer fabric such as duck or canvas or heavy twill plus your usual; i.e. hoops, threads

First you hoop the water soluable stabilizer. If you have the thin kind, you will need 3 to 4 layers in the hoop. Next lay one layer of a tear away backing in the hoop. Do a running stitch the size and shape of the finished patch. This gives you a "placement" line... Tear off the tear away backing outside the stitched line. Using the template, cut a piece of the fabric to fit inside the placement line just sewn... lightly spray the back of this with adhesive to keep it in position until you get some anchor stitches done. Stitch your design...tear off the remaining water soluable stabilizer and you have a great patch. notes: using a template to cut the fabric allows you to do the entire patch without having to cut the excess fabric out of the hoop. Much easier... Hope this helps... TIZME
