Without seeing your project, my sense is that where that particular symbol is on the chart, you use 2 strands of that color, and do the Kalem stitch. But, the rest should be done with whatever your basic stitch is - unless there is another note specifying something different.
Charts typically will have a symbol for a thread/yarn to be done in the basic stitch (tent or XS) and another symbol if that yarn is done with a specialty stitch. So, in this case it sounds as if the designer wants you to do a specific stitch - Kalem - with a specific color of Medici - where you see that symbol charted.
